Dedicated registered social worker with more than 1 year employment experience as a medical Social Worker (MSW) job order under the ministry of health (Government Hospitals) in Tangub City . Self- motivated professional with the commitment to obtain a position which will offer of challenge and responsibilities where my abilities and skills can be fully utilized, and to understand the goals and objectives of your agency. Work experiences: •Performed social work practice in line with the policies , procedures and standards of the institution. •Interviewed and assessed patient in the ward upon admission. • Interviewed admitting client at the emergency room. • Conducted interview to all non philhealth clients. • Identify qualified clients for indigency program. • Clients identified as indigent can be categorize as class C3 and class D. •Assess qualified clients for point of service to fill up CSF and PMRF forms immediately. • Entry Client's data from admission logbook to medical Social services registry book. • Releasing Point of Service (POS) certificate to POS member. • Tagging and posting Point Of Service (POS) at their chart . • Encode Claims for medical assistance program. • Participates in teaching and assessing of Junior colleagues . • Promoted family planning to the client whose source of income cannot suffice thier daily needs. • Ensures family members of clients to receive health education to be able to achieve satisfying and productive patterns of living.
