Overview

We are seeking a highly experienced QA Nurse / Clinical Documentation Specialist to support quality assurance, OASIS review, clinical documentation accuracy, and compliance operations within a fast-paced home health environment. This role requires a licensed nurse with prior home health experience, strong familiarity with WellSky or similar EMR systems, and the ability to independently review and manage clinical documentation workflows with minimal training.

The ideal candidate has experience working directly with US-based home health agencies, possesses strong knowledge of Medicare documentation standards, and can assist with maintaining high-quality, compliant patient records. This position plays a key role in supporting operational efficiency, reducing documentation bottlenecks, and ensuring timely clinical workflow completion.


Key Responsibilities

Quality Assurance & Clinical Documentation Review

  • Review clinical documentation for accuracy, completeness, and regulatory compliance.

  • Perform QA review of OASIS assessments and related home health documentation.

  • Ensure documentation aligns with Medicare guidelines and agency standards.

  • Identify documentation discrepancies, inconsistencies, and missing information.

  • Communicate documentation corrections and follow-up needs with clinicians and internal staff.

  • Monitor chart completion timelines and assist with reducing outstanding documentation backlogs.

  • Verify appropriate documentation supporting patient eligibility and skilled need.

  • Review physician orders, plan of care documentation, and certification paperwork for accuracy.

  • Support timely chart auditing and quality control processes.

Clinical Operations Support

  • Assist with clinical workflow coordination and EMR task management.

  • Support intake, scheduling, or authorization workflows during periods of operational overflow when needed.

  • Upload and organize clinical records, hospital documentation, lab results, and physician paperwork.

  • Monitor pending tasks within WellSky and ensure timely follow-up and completion.

  • Track unsigned orders, expiring certifications, and incomplete documentation items.

  • Assist with interdisciplinary communication and coordination between office and field staff.

  • Support agency readiness for audits, surveys, and compliance reviews.

Compliance & Administrative Support

  • Help maintain audit-ready patient records and organized documentation systems.

  • Assist leadership with quality improvement initiatives and compliance monitoring.

  • Ensure HIPAA compliance and maintain strict confidentiality standards.

  • Support implementation of agency workflows, SOPs, and documentation processes.

  • Maintain professionalism and accuracy while working independently in a remote environment.


Preferred Skills & Qualifications

  • Active RN license required.

  • Prior home health QA and OASIS review experience required.

  • Experience working with US-based home health agencies required.

  • Strong proficiency with WellSky or similar home health EMR platforms required.

  • Strong understanding of Medicare home health documentation and compliance standards.

  • Experience reviewing physician orders, plan of care documentation, and certification workflows.

  • Excellent organizational skills and strong attention to detail.

  • Ability to work independently and manage multiple priorities efficiently.

  • Strong written and verbal English communication skills required.

  • Ability to work US daytime hours consistently.


Nice-to-Have (Bonus Skills)

  • Experience supporting agencies with large census volumes.

  • Experience assisting with scheduling, intake, or authorization workflows.

  • Experience with survey readiness and compliance audits.

  • Familiarity with workflow optimization and operational process improvement.

  • Leadership or clinical coordination experience within a home health setting.
